What it does
Extort lets you tax your opponents whenever you cast a spell. As you cast any spell, you may pay
for each instance of extort on your battlefield; if you do, each opponent loses 1 life and you gain that much life. This triggers off any spell you cast, not just the permanent with extort.
The big nuance: extort triggers on the cast, so it goes on the stack above the spell you're casting and resolves first. You can pay the hybrid mana with either white or black mana, and you can pay it multiple times if you have several extort creatures. Importantly, you can use mana from the spell's own cost leftovers or floating mana, but you must pay when the trigger resolves.
In Commander, extort scales beautifully—it drains all opponents at once, so a board of extort creatures can swing a multiplayer game's life totals dramatically over time, especially in spell-heavy decks.
