What it does
Graft is an old keyword from Dissension that appears on creatures with +1/+1 counters. A creature with Graft N enters the battlefield with N +1/+1 counters on it. Whenever another creature enters the battlefield, you may move a +1/+1 counter from the Graft creature onto that new creature.
The common mistake: the Graft creature doesn't get bigger from its own ability—moving counters only shrinks it to pump others. Also, it triggers on any creature entering, including opponents' creatures, and the ability is optional (you choose whether to move a counter and onto which valid creature). Once all counters are gone, the trigger does nothing.
In Commander, Graft is fairly niche but shines in +1/+1 counter decks, letting you redistribute counters onto better targets, fuel synergies, or build up evasive threats over multiple turns.
